Jacqui Cairns is our Community Link Worker, based in the practice every Friday. She offers 30-minute appointments to discuss social issues impacting your life, providing practical support and advice, signposting to local and national services, and community-based groups and activities.
This includes issues such as Bereavement, New diagnosis, Long-term conditions (e.g dementia, autism etc), Linking patients to Mental Health and Wellbeing services, Housing issues ( e.g Homelessness, medical needs assessment), Fuel Poverty, Caring responsibilities, Loneliness and isolation, Relationship issues, Children issues (e.g bullying/social exclusion/behaviour problems), Change of life circumstances (e.g loss of a job, moving house), Marginalised groups (e.g Asylum Seekers, LGBTQ+). This list is not exhaustive but some of the most popular reasons for referral.
